This pilot project is currently in 10 service areas within LA County. Click on the CPHT Locator on the right to type in your home address and see if you live in one of our service areas. For details on each team, scroll to the bottom of this page.What should I expect?
If you live in a service area, a Community Health Worker will invite you to an in-person household assessment and offer health services based on your responses. You can also join community meetings to learn about CPHT and get more involved.

Advisory committees of residents and partners guide planning, implementation, and evaluation. This builds lasting community relationships, empowers residents to shape health priorities, and improves outcomes through collaborative action.

These partners may attend meetings, help identify health priorities, offer ongoing input, and take on leadership roles--including serving on Advisory Committees to support planning and evaluation of Community Action Plans.
