Diabetes Coalition

The goal of the coalition is to expand access and participation in the National Diabetes Prevention Program (National DPP) and Diabetes Self-Management Education and Support (DSMES) programs.

The coalition brings together key stakeholders in diabetes prevention who have been working towards reducing diabetes prevalence. The coalition consists of over 50 organizations from various sectors:

  • Health System Partners
  • Community - Based Organizations
  • Local National Diabetes Prevention Program (National DPP) providers
  • Academic Partners
  • Health Sector Practitioners
  • Health Plans and Non-profit Organizations

Every Los Angeles County resident has access to high quality diabetes prevention, management, and supportive services.


Our Mission

The Los Angeles Diabetes County Coalition fosters a network of sustainable programs that support Los Angeles County residents' health, particularly among high-risk populations, by helping them prevent and manage diabetes.
