Below you will find guidance on mandatory HIV and STD reporting and disease specific resources on HIV and STD prevention and management, including primers on sexual history taking and STD screening as well as treatment guidelines and diagnosis and treatment resources. Available in this section are downloadable reporting forms, surveillance reports, and guidance on  how to contact program staff for information on reports from our epidemiology unit and patient syphilis titer history and STD management consultation.

Please remember to report your HIV and STD cases to us! It is critically important (and required by law) that you report all cases of HIV, chlamydia (including LGV), gonorrhea, syphilis, PID, and chancroid. The reporting of HIV and STD cases does not require patient consent and does not contradict the Health Insurance Portability and Accountability Act (HIPAA) Privacy Rule.

  • STD/Hotline (for patients): 1-800-758-0880. Counselors available from 7am-5pm, M-F. Automated 24 hours a day, 7 days a week to provide: information for STD clinics and HIV testing locations, and to order free condom delivery (10 condoms/pkg/request).
  • HIV Post-exposure HIV Prevention (PEP) Hotlines PEPline. National Clinicians Post-Exposure Prophylaxis: 1-888-448-4911 (24 hours a day, 7 days a week)
  • Warmline-National HIV Telephone Consultation Service: 1-800-933-3413 (M-F 6am-5pm PST; M-F, Voicemail 24 hours a day)
