Study Protocol

IRB applications must include a protocol that follows the template below in order to be considered for review. An example protocol from a past application is included for your reference.

HIPAA Authorization

If your project will obtain HIPAA authorization, you must use the following HIPAA Authorization form. If you have any questions regarding whether your data includes Protected Health Information (PHI), please review your institution's HIPAA policy and/or contact the Privacy Officer at your institution. If your project will request a waiver of HIPAA Authorization, the request must be submitted with an IRB application for review. Waivers of HIPAA Authorization will only be granted if adequate justification is provided with the IRB application.
