If you started your vaccination as a resident of a LTCF but left the facility, you can make an appointment for your final dose at a location on this webpage. Be sure to check that it offers the same kind of vaccine as your first dose.

There are many ways to show that you are eligible to be vaccinated. Below, is a list of documents that will be accepted. This list is not complete and other documents may be accepted unless otherwise stated.
You do not need to show government issued ID and you do not need to be a US citizen to get a vaccine.
Consent for minors: Minors (age 16 and 17) who meet the eligibility criteria can receive the Pfizer vaccine. Minors must be accompanied by a parent or guardian who can provide consent for vaccination. The Moderna and Janssen vaccines can only be given to people age 18 years and older
