2024-2025 Program Announcement

The Los Angeles County (LAC) Department of Public Health (DPH) continues our Skilled Nursing Facility (SNF) Honors Program. The SNF Honors Program is designed to provide special recognition to facilities for outstanding performance in public health engagement, vaccination coverage, infection prevention and control (IPC) program structure, and preventive action. The SNF Honors Program cycles annually with criteria for completion available from August through May of the Program season.

Criteria

LAC DPH will honor all LAC SNFs for achieving and/or surpassing the following metrics among 4 categories: Public Health Engagement, Vaccination Coverage, IPC Program Structure, and Preventive Action.

SNFs must meet the criteria specific to the category to be honored and are eligible to be recognized in one, two, or even all four categories, for the program year.

Category 1: Public Health Engagement
SNFs must meet ALL of the following:
  • Update their facility contact information annually, using the DPH SNF Contact Form. This is to be completed by May 2025
  • Ask an IP Series
    • Attend 3 sessions in 2024 AND 3 in 2025
  • Participate/engage in the Patient Safety Information Exchange (PSIE) registry

Category 2: Vaccination Coverage
SNFs must meet at least 2 of the following:
  • HCP Influenza vaccine coverage of 90% or higher
  • Resident COVID-19 vaccine coverage of 80% or higher
  • HCP COVID-19 vaccine coverage of 80% or higher

Category 3: Infection Prevention and Control Program Structure
SNFs must meet ALL of the following:
  • Have at least 1 Infection Preventionist (IP) who is certified by the Certification Board of Infection Control and Epidemiology (CBIC). Certified is defined as holding any of the following nationally recognized credentials in infection control:
    • a-IPC: Associate – Infection Prevention and Control
    • CIC: Certification in Infection Control
    • LTC-CIP: Long-Term Care Certification in Infection Prevention
    • BCIDP: Board Certified Infectious Disease Pharmacist
  • Have at least 1 IP who has been employed at the SNF in the Infection Preventionist role for at least 1 year. This does not have to be the same IP as above.

Category 4: Preventive Action
SNFs must meet at least 2 of the following:
  • Complete the NHSN Annual Survey for 2024 no later than May 2025
  • Conduct 1 ICAR visit: ICAR webpage
  • Submit 1 completed A3 template the facility completed for a Quality Assurance and Performance Improvement project
    • A3 template and instructions found in the resource section of the TNT Website
  • Complete 1 prospective MDRO PPS
