About the
Program:
The
TNT program is an
all-inclusive,
financially supported
collaboration between
the Los Angeles County Department of
Public Health (LAC DPH) and
LAC Skilled Nursing
Facilities (SNFs) with
the overall goal of
improving safety and
care for residents,
staff, and visitors.
This program aims to
empower SNF staff to
initiate and maintain
quality improvement
projects within their
facilities, leading to
high quality care for
their residents and
family satisfaction. LA
County SNFs will be
trained in a variety of
areas within quality
assurance and
performance improvement
(QAPI) and infection
prevention and control
(IPC). SNFs will be led
through the foundations
of QAPI and IPC, then
bridged into developing
their own sustainable
QAPI and IPC projects.
All LAC SNFs (including
Pasadena) are eligible
to participate in the
TNT Program.

Enrollment Requirements:
Complete
and
submit
the
Facility
Commitment
Form.
In the
form,
designate
staff,
up to 3
individuals,
to
participate
in the
program.
Form
must
have
been
submitted
no later
than
June
21st,
2022.
All
designated
staff
must
complete
the
Participant
Commitment
Form
that
they
receive
via
email
after
the
Facility
Form is
submitted.
Learning Objectives:
Identify,
analyze,
and list
gaps in
facility
IPC
practices.
Develop
and
implement
a
sustainable
QAPI
project
based on
identified
IPC
gaps.
Empower
a
variety
of
facility
staff to
participate
in QAPI
projects.

Improve facility-wide hand hygiene adherence rates.
The TNT Program hosts weekly Didactic sessions in a variety of areas within quality assurance and performance improvement and infection prevention and control for Skilled Nursing Facilities in Los Angeles County that have enrolled to participate in the program.
The materials for these sessions are available below for all interested parties who would like to review them. However, ONLY designated participants from enrolled facilities are eligible to receive Certificates of Completion or Continuing Education Units (CEUs) after successful completion of the post-session evaluation quiz. All individuals who are not Designated Participants from already enrolled facilities are not eligible to complete the Quiz and will not receive Certificates of Completion or CEUs, but are welcome to review all the information provided below.
