Welcome to the Los Angeles County (LAC) Department of Public Health (DPH) COVID-19 Dashboard for Skilled Nursing Facilities (SNFs). This dashboard provides self-reported weekly information on the number of COVID-19 tests performed, persons diagnosed, and COVID vaccination coverage in LAC SNFs. The data are collected through surveys facilitated by the California Department of Public Health (CDPH). Page 1 includes summary data on 340 SNFs in LAC excluding Long Beach and Pasadena and Page 2 provides facility-specific data including the jurisdictions of Long Beach and Pasadena.
1 23 facilities are in unincorporated areas and are assigned to their nearest city. Some cities listed are neighborhoods within the City of Los Angeles. Thus data for the selection "Los Angeles" excludes facilities in those LA City neighborhoods.
2 Facilities with more than a 7 day supply of each of the following types of Personal Protective Equipment: Eye Protection, Gloves, Gowns, N95 Masks, Surgical Masks; see below for the definition of PPE shortages.

Current LAC SNF guidance states that routine screening testing of residents is no longer required or recommended regardless of vaccination status, except in specific circumstances.

11 Per AFL 20-52, CDPH requires all SNFs to develop and implement a CDPH approved COVID-19 mitigation plan (MP). Beginning in July 2020, CDPH began conducting periodic site visits to assess MP implementation. Links to the CDPH reports from the first and second MP implementation assessments have been provided. Please note, the date listed reflects the start date of the assessment visit and may differ from the final report date. Due to reporting lags, the final report may not yet be posted. Please check back periodically if this is the case.
12 These data reflect the percentage of fully vaccinated staff and residents among those in the facility during the reporting week. Staff members who work at multiple facilities might be counted more than once in the overall coverage estimate on page 1
California Department of Public Health (CDPH) Weekly Survey of COVID-19 SNFs: This is a weekly electronic self-report survey of all SNFs in California. Dashboard data from this survey include: 1) SNF names, addresses, contact information, license numbers, and numbers of licensed beds, 2) average weekly numbers of SNF staff and residents, 3) weekly numbers of COVID-19 tests performed among SNF staff and residents, and 4) numbers of positive staff and resident results received during the survey week, and 5) COVID-19 vaccination coverage among staff and residents.
CDPH Daily 1-2-3 COVID-19 survey in SNFs: This is a daily electronic self-report survey of all SNFs in the California. Dashboard data from this survey include: 1) cumulative COVID-19 cases among SNF staff and residents since January 2020, 2) new COVID-19 deaths among SNF residents and staff during the reporting week, 3) cumulative COVID-19 deaths among SNF staff and residents since January 2020, 4) SNF staffing shortages during reporting week, and 5) shortages of personal protective equipment (PPE) during reporting week.
Los Angeles County Department of Public Health: For facilities that missed submitting their weekly CDPH survey, a local LACDPH survey is sent to supplement CDPH data, therefore dashboard data from different sources will not always match.

Please note, known and previously positive staff and residents, or those with pending test results from the previous week do not need to be retested, therefore it is likely that many facilities will report testing less than 100% of staff and residents in a given week.
Staffing and PPE shortages:
A SNF staffing shortage is defined as any “yes” response during the reporting week to the CMS survey question “Do you have an urgent staffing need over the next 48 hours?”. A SNF PPE shortage is defined as any report of less than or equal to a 7-day supply of eye protection, gloves, gowns, N95 masks or surgical masks during the reporting week.
