LA County Department of Public Health - Substance Abuse Prevention and Control - Fiscal Year 2015-16 DMC Annual Cost Report Forms and Instructions

The State just released the revised DMC cost report forms to the County and they can be downloaded from our website. The due date for providers to submit their DMC cost report to County is March 30, 2017.
Units Reconciliation Report - This report was e-mailed to each provider during the month of November 2016.
The following two documents/files must be submitted by your Agency to SAPC-Cost Reporting Unit by March 30, 2017:
1. "Provider Information and Certification" with original signature.
2. Original Cost Report (by Site and Modality).

IMPORTANT:
All cost reports must be submitted by Site. No exception.

If the cost reports are not received by the due date, your agency will be listed as delinquent.
After March 30, 2017, a stop payment will be placed on current billings for delinquent providers. The Stop payment order will remain until such time cost reports are received.
Do not staple or bind cost reports.
