APPOINTMENTS (both scheduled and walk-ins) ARE AVAILABLE ONLY ON TUESDAYS, WEDNESDAYS, AND THURSDAYS FROM 8:00 AM TO 3:30 P.M.

Appointments are STRONGLY recommended.  Walk-ins are only accepted as time allows and wait times may be very long.  No appointments or walk-ins will be processed after 3:30 pm.

You must obtain the following before coming to apply for a card:

1. A written recommendation from your physician and make sure there is a copy in your medical file.
2. A Medical Marijuana Program Application/Renewal form (English /Spanish).
3. Call to schedule your appointment at 1-866-621-2204. Walk-ins are only accepted as time allows and wait times can be very long.

Please bring the following:

1. The original written recommendation from your physician on official letterhead, prescription pad, or Written Documentation of Patient's Medical Record. (English/Spanish)
2. A completed original Medical Marijuana Program Application/Renewal Form (English /Spanish).
3. A government-issued photo identification (i.e. California State Driver's License, a California State ID Card, United States Passport, Veteran's Administration ID Card).
4. The following TWO current forms for proof of Residency in Los Angeles County are required.
   1. The current photo IDs listed above.
   2.  A current utility bill or California motor vehicle registration with your name and current address, mortgage payment receipt, cellular telephone bill, voter registration, bank statement or automobile, property, life or health insurance documents. P.O. Box address is not acceptable for address verification.
5. Regular application fee is $153.00  or ($76.50 for approved Medi-Cal participants).  Payment can be made by cash, check, or money order and payable to "Los Angeles County Dept of Public Health". If you choose to pay by cash, exact change is appreciated. Application fees are non-refundable.

1. The physician's license will be verified with the Medical Board of California or the Osteopathic Medical Board of California.
2. The physician's letter of recommendation will be verified with the physician.  The physician listed on the Original Letter of Recommendation will be the only signature accepted for authorization.
3. You can choose to have your Medical Marijuana Identification Card (MMIC) sent by Certified Mail or schedule an appointment for the card pick-up.  It can take up to 35 days to receive your MMIC if the application is complete.  If a MMIC gets lost in the mail, you will need to reapply.
