Order Birth Certificates

NOTICE OF FEE INCREASE EFFECTIVE JANUARY 1, 2012:  ASSEMBLY BILL 1053, signed by Governor Brown and Chaptered on October 2, 2011 (Chapter 203, Statutes of 2011), authorized an increase of $2.00 for each certified copy of a birth, death or fetal death record.

Fees: $21.00 per certified copy (as of January 1, 2012)

Birth certificates can be obtained from several different local and state vital records offices depending on where the birth took place.

The Los Angeles County Vital Records Office is able to provide certificates for all births occurring in Los Angeles County from 1964 to the current year.  Births prior to 1964 must be obtained at the Registrar-Recorder/County Clerk's office.  The Registrar-Recorder/County Clerk's and any of their satellite offices can also provide birth certificates approximately 5 weeks after the date of birth.

By Mail

To order a copy of a certificate for a birth that occurred between 1964-present, please fill out two forms:

    1. Application for a Birth Record

    2. Certificate of Acknowledgement (must be notarized if sent by mail)

Mail check (DO NOT SEND CASH), order form, and a notarized certificate of acknowledgement to:
