Greater San Gabriel Valley Hospital Collaborative
The Greater San Gabriel Valley
Hospital Collaborative began meeting in mid-2018. The
Hospital Collaborative is an initiative of and
facilitated by the Health Consortium of Greater San
Gabriel Valley (Health Consortium). The mission of the
Health Consortium is to strengthen the health care
safety net and optimize seamless access to high quality
physical and behavioral health services in the Greater
San Gabriel Valley (SGV). The “Greater San Gabriel
Valley” includes both the San Gabriel and Pomona
Valleys, stretching from Pasadena to Pomona and
incorporating the geographic area defined by Los Angeles
County as Service Planning Area (SPA) 3.
The Greater San Gabriel Valley
Hospital Collaborative serves to both (a) work
collaboratively to streamline and coordinate data
collection for Community Health Needs Assessments
(CHNAs) across the hospitals; and (b) develop
coordinated strategies to address regional health
priorities. Programs initiated by the Hospital
Collaborative include the SPA 3 Patient Navigation
program and the Food for All San Gabriel Valley program.
These programs are designed to provide navigation
support to vulnerable hospital patients upon discharge
to address needs associated with homelessness and food
insecurity. The six nonprofit hospitals that comprise the
Hospital Collaborative are City of Hope, Emanate Health,
Huntington Hospital, Kaiser Permanente Baldwin Park, USC
Arcadia Hospital and Pomona Valley Hospital Medical
Center. In addition to the nonprofit hospitals, the
Hospital Collaborative also includes the two local
public health departments that serve this geographic
area - L.A. County Department of Public Health and
Pasadena Public Health Department .
The Hospital Collaborative is funded
by UniHealth Foundation, United Way of Greater Los
Angeles, and grants/contributions from our hospital
partners.

Valley Community Health Snapshot Report
The Center for Nonprofit Management (CNM) has provided technical assistance to the Hospital
Collaborative relative to the coordinated CHNA process,
including review of data collected by all the hospitals.
CNM worked together with the Pasadena Public Health
Department and Health Consortium of Greater SGV to
prepare the Greater San Gabriel Valley Community Health
Snapshot report, released on April 4,
2023 at a community presentation of the findings at City
of Hope. The Snapshot Report provides a high-level
overview of data that describes the area’s population
and five priority issue areas in the Greater SGV,
including: Behavioral and Mental Health, Health Care
Affordability and Accessibility, Housing and
Homelessness, Economic and Food Insecurity, and Chronic
Conditions.
