Asian & Pacific Islander American Health Forum
The Asian & Pacific Islander American Health Forum (APIAHF) is a national advocacy organization dedicated to promoting policy, program, and research efforts to improve the health and well-being of Asian American and Pacific Islander (AAPI) communities. Founded in 1986, the Health Forum approaches activities with the philosophy of coalition-building and developing capacity within local AAPI communities. We advocate on health issues of significance to AAPI communities, conduct community-based technical assistance and training, provide health and U.S. Census data analysis and information dissemination, and convene regional and national conferences on AAPI health. APIAHF's Chronic Diseases Program includes the funded programs of tobacco control and cancer survivorships in addition to past projects in cardiovascular disease. APIAHF continues to address issues that are connected to the disease areas presently being addressed. California Food Policy Advocates (CFPA) is a statewide non-profit organization whose mission is to improve the health and well-being of low-income Californians by improving their access to nutritious, affordable food. COPE's TEACH program
Established in 1995, COPE is a 501(c)(3) nonprofit health care organization whose mission is to Make Communities Healthier. COPE provides services through three division areas: The Health Workforce Transformation division, one of AmeriCorps 51 Most Innovative Programs in the country, revolutionizes the way hospitals and health care systems recruit, train, retain and mentor nursing and allied health care staff. Health Systems Improvement was developed as a partnership with the LAC+USC Health Network and 5 community clinics to identify and enroll frequent users of hospital, inpatient and emergency department services into a coordinated care management program. The Family Health & Education division offers free after school mentoring, tutoring and health education to low income, disadvantaged, at-risk youth and their families in its TEACH program. This division also offers Healthy Babies, a comprehensive perinatal service program designed to educate and support low-income, at-risk pregnant women. Keck Diabetes Prevention Initiative
Responding to the current epidemic of diabetes and obesity, the Keck Diabetes Prevention Initiative (KDPI) will develop, implement, and evaluate a ten-year initiative aimed at reducing the prevalence of obesity and diabetes in two communities in Los Angeles. Based on County data, the selected communities have been designated as medically underserved, having the highest incidence of diabetes-related deaths in Los Angeles, and are characterized by high rates of obesity and poverty. The two target communities identified are South Los Angeles and East Los Angeles, specifically in and around the zip codes of 90059 and 90022 respectively. The KDPI is divided into three phases. During Phase I (2004 - 2006) the Initiative aims to: 1) Analyze existing data on the prevalence of diabetes and associated health complications; 2) Gather information from community residents and leaders to attain a grounded understanding of the social, cultural and historical consumption of healthy food consumption and physical activity through key expert interviews, community advisory boards, focus groups and town hall meetings; 3) Map community resources and barriers to healthy eating and physical activity to identify areas where we can build on existing efforts and identify areas of need. During Phase II and III (2006 - 2013) KDPI will: 1) Determine and implement interventions based on community needs; 2) Identify outcomes measurements to be used in the intervention phase and develop evaluation strategies, methods, and protocols; and 3) Identify and partner with additional funding sources to secure resources for interventions; and 4) Disseminate formative research project findings and lessons learned. Long Beach USD Nutrition Network/Health Education
The Long Beach USD serves more than 92,000 students in 95 public schools in the cities of Long Beach, Lakewood, Signal Hill, and Avalon on Catalina Island. The third largest school district in California, it serves the most diverse large city in the United States, according to Census 2000, with 46 different languages spoken by local students. The district employs more than 8,000 people, making it the largest employer in Long Beach. The school district was named the 2003-04 national winner of the $500,000 Broad Prize for Urban Education, recognizing America's best urban school system for increasing student achievement. The district believes the health of students and their success in school are intimately linked. Students must be healthy to be able to learn and students must be educated to keep themselves healthy. Our mission is to develop health literacy in all students. LBUSD is committed to standards-based health education. The LBUSD addresses nutrition education and physical activity promotion through a coordinated school health system that is consistent with CDC's model and is described in the Health Framework for California Public Schools and the LBUSD Health Education Content Standards. Building upon existing efforts, LBUSD will utilize California Nutrition Network funds to increase the likelihood that low-income students and their families will consume five fruits and vegetables daily, participate in physical activity (60 minutes per day for youth, 30 minutes per day for adults), and participate in food assistance programs. A Project Teacher and two Child Nutrition Specialists provide teacher training and parent education classes, while providing coordination support for the Harvest of the Month program and Nutrition Advisory Councils at low income schools. Project Angel Food
The Project Angel Food mission is to nourish the body and spirit of men, women, and children affected by HIV/AIDS and other serious illnesses. Project Angel Food delivers free meals prepared with love. We act out of a sense of urgency because hunger and illness do not wait. The Moveable Feast
The Moveable Feast is a nutrition education/food preparation program currently operating in two school districts: Baldwin Park Unified and San Gabriel Unified. In Baldwin Park, six classes of fourth grade students from three elementary schools are bussed to the Baldwin Park Community Garden to plant vegetables with the gardening teacher and then transform their produce into healthy child-friendly recipes, which each child makes and tastes, under the direction of Linda Hahn, Program Director. In San Gabriel Unified, seventh grade students in health classes and English Language Learners at Jefferson Middle School learn nutrition principles, while making and preparing healthy recipes. Adults learning English in the Community Based English Tutoring (CBET) program in San Gabriel also learn "English Through the Language of Food" from California State University, Los Angeles dietetic interns, under the supervision of Linda Hahn.
