Champions for Change - Healthy Communities Initiative

The Champions for Change Healthy Communities Initiative is a Supplemental Nutrition Assistance Program Education (SNAP-Ed) grant that aims to reduce obesity and chronic disease among low-income populations through a coordinated approach:

  • Nutrition education and physical activity promotion
  • Community/stakeholder engagement and mobilization
  • Policy, systems and environmental change (PSE)

Twenty-four agencies across Los Angeles County are awarded the Champions for Change grant for three years (2017- 2019) to implement obesity and chronic disease prevention strategies that is aligned with the social ecological model and policy, systems, and environmental (PSE) change.
