The CHDP Referral Directory was created to make it easier for CHDP providers to refer
children who have fee-for-service Medi-Cal through the CHDP Program to a Medi-Cal specialist,
dentist or vision care provider.

Disclaimer: These providers were asked if they accepted new fee-for-service Medi-Cal
patients and would be willing to be a part of our referral list. All of the providers on this
list have agreed to participate in our referral network.
To the best of our knowledge, the information about these providers is correct; however, it is
subject to change. Before referring a CHDP patient to one of these providers, please call to
confirm that the provider still accepts Medi-Cal.
