Reporting E-cigarette, or Vaping, Product Use-Associated Lung Injury (EVALI)

What to report:

For case-definition, see the California Department of Public Health's Health Alert on EVALI. 

How to report:

Report EVALI as an occurrence of an unusual disease on the Los Angeles County Department of Public Health Confidential Morbidity Report (CMR) Form.

Fax the form to the Los Angeles County Department of Public Health at 888-397-3778 or 213-482-5508 or send via secure email to RPU@ph.lacounty.gov.
