    Beginning December 15, 2013, there will be a fee for all immunizations: $15 per child or adult or $30 for a family with 2 or more children. If you have health insurance or a regular health care provider who covers immunizations, please contact your regular health care provider for the immunizations. If you cannot pay this fee, please let the clerk know. No child will be denied vaccines due to inability to pay.

It also provides information on deaths from a wide range of health conditions, including chronic diseases, such as heart disease and emphysema; infectious diseases, such as pneumonia/influenza and HIV; lifestyle-related conditions, such as diabetes and lung cancer; and intentional and unintentional injuries.

    This report features data about health-related behaviors, such as access to medical care, dental care, and health insurance; utilization of preventive services; rates of chronic disease, infectious disease, and injury; and perceptions about health.
