California Department of Public Health digital COVID-19 vaccine records are now available.
The record can be saved in Google Pay on an Android phone or the Apple Health app.
All vaccine records are free. Beware of vaccine record scams.
There are several types of vaccination record:
*You can show the card, a photo of the card as a separate document, or a photo of the card stored on a phone or electronic device.
To prove that you have been vaccinated, you must show a photo ID in addition to your vaccination record. The record should include your name, the date(s) that you were vaccinated, and the type of vaccine that you received.
You are considered fully vaccinated against COVID-19 two weeks after:

Everyone in the US should be given a CDC COVID-19 Vaccination Record Card when they are vaccinated. Please keep it safe. Consider taking a photo or making a photocopy of it. If you did not get a white card, or you lost it, you can print a copy of your digital COVID-19 vaccine record (see below).
You can request a digital COVID-19 Vaccination Record if you were vaccinated in California (unless you were vaccinated at a federal facility – see below). Visit myvaccinerecord.cdph.ca.gov to request your record. Be sure to use the email or cell phone number you used when you received your vaccine. It takes less than a minute to complete the request and your record is sent by email or text within seconds.
The digital record includes a QR code that can be scanned by a SMART Health Card reader. You can save your digital vaccine record to Google Pay if you have Android phone version 5 and Google Play Services version 21.18 or above. If you have an iPhone with the iOS 15 operating system you can also save your digital vaccine record to the Apple Health app. You will need to use your Safari web browser to complete the process. Another option is to take a screenshot of your record or print it out.
If you were vaccinated by a federal facility (e.g., Department of Defense, Indian Health Services or Veterans Affairs), you must request a vaccine record from the facility directly. This is because the California Immunization Registry (CAIR) does not hold information on vaccines given in these facilities.
The Los Angeles County Department of Public Health partnered with Healthvana in December of 2020 to provide digital vaccination records. These are available to residents of LA County (including Pasadena and Long Beach) who have received at least one dose of vaccine (regardless of where they were vaccinated) and anyone who has received at least one dose in LA County (regardless of where they live).
If you received a vaccine at one of these locations, you should receive an email from vaccine@Healthvana.com and/or a text message from Healthvana with a secure link (most text messages will display as 37586). When you click the link in the email or text, you will be asked to enter your first name, last name, and date of birth to confirm your identity. You can then view your digital vaccination record. You also have the option to download it to a digital wallet (Apple Wallet for iPhones and Google Pay for Android devices).
Due to the high volume of vaccination records being processed, there may be a delay in getting your digital record from Healthvana. If you don’t get the email or text within 1 month of being vaccinated, visit the healthvana.com webpage and select “contact” in the menu to request your record. Please note that it is not currently possible to upload a self-reported vaccine record into Healthvana unless you already have a Healthvana record of your first dose (see below).
If your 2nd dose is missing from your Healthvana record:
You can complete your digital vaccination record by uploading an image of your white CDC vaccination card as proof of your 2nd dose. To do this:
The Los Angeles City Fire Department partnered with Carbon Health to provide vaccination at multiple locations in Los Angeles. If you were vaccinated by Carbon Health at any location you can get a digital vaccine record healthpass through the Carbon Health mobile app mobile app or website. The healthpass can be downloaded on the iPhone Apple Wallet. You need to log in to access your healthpass.
VaxYes, CLEAR Health Pass, and Common Pass are other forms of digital vaccination records that can be used as proof of vaccination in Los Angeles County.
Vaccine passport scams are being used to steal people’s money or identity!
Right now, there are no plans to create a national COVID-19 vaccine verification app, certificate or passport.
