What to
report
California law requires healthcare providers to report:
- Suspected or confirmed cases of specified communicable diseases
- All disease outbreaks
- Any unusual disease
Unusual diseases include:
- Rare diseases
- Newly apparent or emerging diseases
- Syndromes of uncertain etiology that may be due to a transmissible infectious agent or microbial toxin.

Where to report
- Residents of LA County: Report to the Los Angeles County Department of Public Health.

When to report
Some diseases, all outbreaks, and unusual diseases require urgent reporting. See page 2 of
LA County Provider Reportable Diseases and Conditions List for reporting priority by condition.
URGENT REPORTING
Report suspected or confirmed cases immediately by telephone.
- Weekdays 8:30am–5pm, call 213-240-7941.
- After-hours, call 213-974-1234 and ask for the physician on call.
NON-URGENT REPORTING
Depending on the disease or condition, report within 1 working day
or 7 calendar days of identification by phone, email, mail, or fax.
